Providing public information

Public information of PE “The State Expert Center MoH Ukraine” is provided on request for information inquiry.

According to the Law "About the access to public information", the information is considered to be public if reflected and documented by any means and at any media that has been obtained and generated in the process of fulfilling obligations by authorized subjects, being envisaged by the current legislation, or which is in possession of authorized subjects.

The individual request for information shall be made in optional form, including:

1) Name and  last name of requester, postal address or e-mail and telephone number;

2) Description of information which requester would like to receive. (Type, name, essential elements or contents of a document pertinent to which request has been made);

3) Signature and date.
